Does this fit over the stoma pouch and dies it have back support

The belt goes round you like a corset.
Very thick velcro (top to bottom) allows you to adjust the waist size. If you're on the border between sizes go for the smaller size, as a little tighter is better.
There is NO back support, but the whole thing is approx 8" or 20cm tall, made of elasticated material. If you wear it for a while, it will concertina down a little to fit your contours.
The white rectangle (shown in amazon picture) is harder plastic. The stoma pouch goes through the hole, and dangles in front of the white bit.
This gives decent all-around pressure on the stoma-bag-adhesive and underlying skin - meaning you are less likely to suffer blow-outs.
You can wear it in bed for extra reassurance.
I found that if you have a bag cover (for modesty) you can wear this on the outside. I found I had to attach velcro to the white plastic bit and the bag cover to prevent it slipping down. May be better with other bag covers.
They can be machine washed. They dry quickly. They tend to discolour a little under the bag.
Well worth the money. 
If you have a para-stomal-hernia already then it will help to control it.
If you don't yet have one, then be aware 50%-odd of us will develop one, and this will at least help to prevent one. Prevention is better than cure.

Many thanks for your question and concern. In the second picture,there is the size chart.The suitable waist circumference for size M is 38 to 44 inches, and the suitable waist circumference for size L is 44 to 48 inches.
